You zone in to the instanced Silyssar through Zhisza. Once inside head slightly southeast to the portal and enter. This transports you up to the first temple. Clear / train your way to the portal to the second pyramid then to the portal to the third. Once inside the third pyramid clear all the way down the basement level. Here you will find Empress Saarisz and 4 guards. The guards have identical names except for the last 4 letters. Observe the chants they perform: High, 1st Middle, 2nd Middle, and Low.

When you engage, all 5 will agro. The guards hit for around 1200 and the Empress hits for up to 2500, has AE rampage, and casts 2 spells:

• Grip of Deathly Visions I - single target deagro, drunk, melee slow, 1200 dd, and feign death. 4 curse counters.
• Grip of Mental Terror I - mana drain that also increases spell costs. 4 poison counters.

You need to kill the 4 guards in the order of the chants above and then the Empress. If you kill any out of order they respawn and an additional "resurected snake" adds. Keep them controlled, burn in the proper order, and they go down. If you do wipe, ths dead stay dead so have a rezzer with a campfire nearby to get back easily. When they are all dead EVERYBODY needs to loot a Softly Glowing Runed Pendant!! This is required for progression, if you forget you'll need to redo the entire mission.
